We ensure that patients have convenient access to groundbreaking treatments and innovations that promote their health and overall well-being.

Key Responsibilities:

  1. Facilitates the transfer and acceptance of patients to USA Health hospitals; gathers clinical data pertinent to transfer requests and matches patient needs with facilities that have the appropriate bed availability and service capabilities.
  2. Collaborates with multiple departments to cultivate relationships with referring facilities, physicians, and hospital personnel.
  3. Operates within the transfer call center, responding to inquiries, entering data into a comprehensive database that includes facility service profiles, approved clinical protocols, and established patient care guidelines.
  4. Coordinates the transfer of emergent, urgent, and elective patients from various referral sources, including hospitals, clinics, nursing homes, and physician offices, through a streamlined process to expedite decision-making.
  5. Ensures patient throughput to final destinations with a focus on quality and efficiency, working closely with stakeholders involved in patient flow.
  6. Maintains accurate and concurrent data collection and documentation, facilitating patient movement through the continuum of care.
  7. Triages and prioritizes patient information, effectively communicating clinical presentations to accepting physicians and nursing staff.
  8. Completes all necessary training and demonstrates proficiency in electronic applications related to job functions.
  9. Adheres to regulatory standards such as EMTALA, Ethics and Compliance policies, and quality initiatives, monitoring self-compliance and implementing process improvements as needed.
  10. Engages in professional communication with physicians and external facilities regarding patient transfers and bed placements.
  11. Exhibits regular and punctual attendance, with the ability to work defined schedules and overtime as required.

Qualifications:

High school diploma or equivalent, accompanied by a minimum of one year of relevant experience.
