Our dedicated team, comprising physicians, advanced care providers, nurses, therapists, and researchers, delivers the region's most sophisticated medical services across various facilities, campuses, clinics, and educational settings. We ensure that patients have convenient access to groundbreaking treatments and innovations that enhance their health and overall well-being.
Key Responsibilities:
- Facilitates the transfer and acceptance of patients to USA Health hospitals, gathering clinical information pertinent to transfer requests.
- Matches patient needs with the appropriate facility based on bed availability and service capabilities.
- Collaborates with multiple departments to build strong relationships with referring facilities, physicians, and hospital personnel.
- Operates the transfer call center efficiently, responding to calls and inputting data into a comprehensive database of facility service profiles and clinical protocols.
- Coordinates the transfer of emergent, urgent, and elective patients from various referral sources, including hospitals, clinics, nursing homes, and physician offices.
- Ensures patient throughput to final destinations while maintaining a focus on quality and efficiency.
- Works closely with all parties involved in patient flow to guarantee timely transfers and accurate bed assignments.
- Maintains accurate and concurrent data collection and documentation.
- Facilitates patient movement through the continuum of care, ensuring that the appropriate level of care is determined based on provided clinical information.
- Triage and prioritize patient information, effectively communicating clinical presentations to accepting physicians and nursing staff.
- Completes all required training and demonstrates proficiency in electronic applications related to job functions.
- Adheres to regulatory standards, including EMTALA, Ethics and Compliance policies, and quality initiatives.
- Monitors compliance and implements process improvements to align with regulations and quality standards.
- Communicates with physicians and external facilities regarding patient transfers and potential bed placements.
- Maintains professional communication and customer service standards with internal and external departments.
- Exhibits regular and prompt attendance and is flexible with scheduling, including overtime as necessary.
Qualifications:
High school diploma or equivalent, along with a minimum of one year of relevant experience.
