Job Title: LPN Hospice Caregiver

Job Summary:

As an LPN in our Hospice team, you will provide specialized care to patients with serious illnesses, under the direction of a Registered Nurse (RN) or Medical Doctor (MD). Your responsibilities will include:

  • Performing procedures that require specialized training, such as sterile dressings and cauterizations
  • Participating in the planning and provision of patient care
  • Providing functional direction to ancillary personnel
Requirements:

To be successful in this role, you will need:

  • A current Licensed Practical Nurse (LPN) license
  • Basic Life Support (BLS) certification
  • Health Care Provider certification
  • Arrhythmia certification (preferred)
  • A valid Driver's License (DL) and be at least 21 years of age or older
Essential Functions:

As an LPN in our Hospice team, you will:

  • Provide care for assigned patients, under the supervision of an RN according to established standards and nursing process
  • Perform admission assessments and ongoing re-assessments based on departmental specific policy and procedures
  • Assist in the development of a prioritized interdisciplinary plan of care on each patient
  • Implement and coordinate plan of care utilizing appropriate organizational skills and reporting mechanisms
