LPN Hospice Caregiver

Job Summary:As an LPN in our Hospice team, you will provide specialized care to patients with serious illnesses, under the direction of a Registered Nurse (RN) or Medical Doctor (MD). Your responsibilities will include:
- Performing procedures that require specialized training, such as sterile dressings and cauterizations
- Participating in the planning and provision of patient care
- Providing functional direction to ancillary personnel
To be successful in this role, you will need:
- A current Licensed Practical Nurse (LPN) license
- Basic Life Support (BLS) certification
- Health Care Provider certification
- Arrhythmia certification (preferred)
- A valid Driver's License (DL) and be at least 21 years of age or older
As an LPN in our Hospice team, you will:
- Provide care for assigned patients, under the supervision of an RN according to established standards and nursing process
- Perform admission assessments and ongoing re-assessments based on departmental specific policy and procedures
- Assist in the development of a prioritized interdisciplinary plan of care on each patient
- Implement and coordinate plan of care utilizing appropriate organizational skills and reporting mechanisms
