LPN - Hospice Care Specialist

We are seeking a skilled LPN to join our team in providing high-quality care to patients in need of hospice services. As a LPN, you will work under the supervision of a Registered Nurse to deliver compassionate and personalized care to patients in their homes.
Key Responsibilities- Perform specialized nursing procedures and provide general nursing care to assigned patients
- Participate in the planning and provision of patient care, including admission assessments and ongoing re-assessments
- Implement and coordinate individualized plans of care, including patient teaching and discharge planning
- Administer medications and perform clinical procedures as assigned
- Maintain accurate records and reports, and perform other job-related duties as assigned
- Current Licensed Practical Nurse (LPN) license
- Basic Life Support (BLS) certification
- Valid Driver's License and must be at least 21 years of age or older
- Ability to work in a fast-paced environment and prioritize tasks effectively
