Hospice Support Nurse

We are seeking a reliable and compassionate Licensed Practical Nurse (LPN) to join our team of hospice clinicians in providing end-of-life care to patients in the Portland community. As a locally-owned hospice agency, Tenderly Hospice LLC aims to offer a unique and personalized approach to hospice care.
Responsibilities:- Assist in planning, coordination, and provision of hospice care in accordance with the patient's plan of care.
- Provide excellent hands-on bedside care to achieve maximum comfort and dignity for patients during their end-of-life journey.
- Build trusting relationships with patients and their families, as well as with other care team members involved in care.
- Utilize problem-solving, critical thinking, and social skills to provide holistic care and support.
- Must be a Licensed Practical Nurse licensed by the state with a minimum of 1 year experience in home health, skilled nursing facility, memory care unit, or hospice.
- Must have excellent problem-solving, critical thinking, and social skills.
- Must be able to extend a holistic care experience to achieve maximum comfort and dignity during the patient's end-of-life journey.
- Ability to communicate well, both verbally and in writing.
- Must have the ability to demonstrate self-confidence and positive attitude toward self and others and maintain commitment and enthusiasm to goal achievement.

We are looking to fill a new position: 3 days a week, 5:00pm-8:00am shift.
