We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Adventist Health. As a Hospice LPN, you will provide high-quality patient care and support to individuals in need of end-of-life care.
Key Responsibilities- Provide nursing care to patients in their homes, under the supervision of a Registered Nurse (RN)
- Conduct admission assessments and ongoing re-assessments to develop and implement individualized care plans
- Administer medications and perform clinical procedures as assigned
- Collaborate with the healthcare team to ensure seamless care coordination
- Maintain accurate and detailed records of patient care and progress
- Current LPN license in good standing
- Basic Life Support (BLS) certification
- Valid Driver's License and reliable transportation
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
