Hospice LPN Position

Gracious Care Hospice is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team in providing exceptional care to end-of-life patients in various settings. As a Hospice LPN, you will be responsible for delivering high-quality nursing care, working collaboratively with our multidisciplinary team, and ensuring the best possible quality of life for our patients.
Responsibilities:- Provide skilled nursing care to end-of-life patients in home, assisted living facilities, adult care homes, memory care facilities, nursing homes, and other settings.
- Work collaboratively with our team to develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct thorough assessments, document patient information, and communicate effectively with patients, families, and healthcare team members.
- Participate in on-call/runner duties, responding to patient needs and emergencies promptly.
- Utilize strong problem-solving skills to address patient needs and emergencies, and work effectively in a fast-paced environment.
- Active LPN license in Oregon.
- Experience in nursing, particularly in end-of-life care, home health, and/or hospice.
- Familiarity with home health and hospice regulations and standards.
- Knowledge of anatomy, physiology, and medical terminology.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
