Home Health/Hospice CNA Opportunity

Connected Home Health & Hospice is seeking a compassionate and skilled Home Health/Hospice CNA to provide exceptional care to patients in the Greater Portland area.

About the Role

As a Home Health/Hospice CNA, you will play a vital role in delivering personalized care and support to patients in their homes. Your primary responsibilities will include:

  • Providing personal care and treatment to patients, including assistance with daily living activities and medication management.
  • Preparing and serving meals, as needed.
  • Assisting with range of motion and simple procedures as an extensional therapy service.
  • Taking and recording vital signs, such as temperature, pulse, and blood pressure.
  • Assisting with laundry and addressing patient safety needs.

You will work closely with our interdisciplinary team to develop and implement personalized care plans that meet the unique needs of each patient. Your ability to work independently and demonstrate critical thinking will be essential in this role.

Requirements

To be successful in this role, you will need:

  • CNA licensure in Oregon.
  • At least 1 year of experience in home health or hospice care, preferably.
  • Ability to work independently and demonstrate critical thinking.
  • Current BLS certification from AHA.
  • A valid driver's license and access to a reliable vehicle.
