Job Overview

POSITION SUMMARY:

The Elder Care Outreach Program Manager plays a pivotal role in informing referral sources and potential clients about the Program of All-Inclusive Care for the Elderly (PACE) services and their eligibility criteria. This position entails conducting outreach initiatives, assisting prospects throughout the onboarding journey, and ensuring adherence to regulatory standards. The Manager will also carry out clinical evaluations utilizing the Uniform Assessment System of New York (UAS-NY) while ensuring meticulous documentation.

KEY RESPONSIBILITIES:

  • Establish and nurture relationships with referral partners to meet enrollment objectives.
  • Confirm eligibility for Medicare, Medicaid, and PACE programs.
  • Provide educational sessions on PACE benefits and initiatives.
  • Facilitate program education through diverse channels.
  • Collaborate with various departments to enhance program operations.
  • Conduct pre-enrollment evaluations for potential participants.
  • Maintain accurate records of all interactions and outcomes.
  • Engage in administrative tasks as required.
  • Track key performance indicators and recommend enhancements.
  • Oversee departmental staff and deliver necessary training.
  • Comply with organizational policies and standards.

WORK SCHEDULE: 8:30 AM - 5:30 PM

HOURS PER WEEK: 40

QUALIFICATIONS:

Education: A Bachelor's degree in Nursing or a related field is preferred.

Experience:

  • A minimum of 1 year of experience working with the elderly population.
  • 3-5 years of experience in geriatrics, home care, or a related field.
  • Familiarity with community-based long-term care services.
  • Knowledge of regulatory healthcare standards.
  • Experience with UAS-NY is essential.
  • Ability to work autonomously.

LICENSE: Must possess an active New York State RN license.

ADDITIONAL REQUIREMENTS:

  • Legally authorized to practice in the State.
  • Medical clearance for communicable diseases is required.
  • Strong interpersonal and communication skills are necessary.
  • Excellent organizational skills are essential.
  • Proficiency in computer skills is required.
  • Experience with Care Compass is preferred.

LANGUAGE SKILLS: Bilingual candidates are preferred.
