Job Summary

The Elizabeth Hospice is seeking a skilled and compassionate LVN to provide professional nursing care to patients in their homes or facilities. As a Hospice LVN Continuous Care NOC, you will work under the supervision of an RN to deliver high-quality care and support to patients and their families.

Key Responsibilities
  • Provide professional nursing care to patients in their homes or facilities, under the supervision of an RN.
  • Assist RNs in analyzing situations, anticipating problem areas, and taking appropriate action.
  • Evaluate patient responses to care and make adjustments as needed.
  • Perform licensed nurse skills, such as administering medications and providing wound care.
  • Implement and document teaching plans based on patient and family needs.
  • Document all care provided to patients, including telephone interactions with other staff members.
  • Enter or retrieve data from established computer files.
  • Orient, instruct, and train assigned personnel.
  • Perform phlebotomy as needed.
  • Support the agency in completion of assigned audits and other office duties.
Requirements
  • Vocational Nurse Training.
  • Minimum 12 months of experience as an LVN, with preferred experience in acute care settings, home health, or hospice.
  • Current California LVN license.
  • Familiarity with computerized bedside patient documentation.
  • Ability to function as part of a team and take direction from other team members.
  • Phlebotomy certification a plus.
  • Bi-lingual (Spanish) a plus.
  • Current California driver's license and proof of insurance required.
  • Excellent communication skills with patients, families, and other medical professionals.
  • Ability to remain calm and flexible in stressful situations.
  • Current CPR/BLS Certification required.
  • Current TB test results required.
