We are seeking a skilled Hospice Care Specialist to join our team, providing high-quality care to patients in the community or facility-based settings. Our hospice and palliative care programs provide community or facility-based care to ease the burden of managing a life-limiting condition. Responsibilities:
This role requires a strong nursing background, with a focus on hospice care, and excellent communication and teaching skills. The successful candidate will be responsible for developing and updating care plans as needed, triaging calls outside regular office hours, and providing direct nursing care to patients with home visits. Additionally, they will document nursing care in the computer system and communicate changes in patient condition to primary RN case manager.

Requirements:
To be considered for this role, candidates must have a graduate degree from an accredited school of Nursing, with a minimum of 1-2 years of experience in medical/surgical, Community Health, and/or Hospice/oncology Nursing. They must also possess good communication, listening, and teaching skills, as well as knowledge of Word, Windows, and Excel. A driver's license and car are required for field-based work.
