Job Description:
As a Hospice Care Specialist at United Hospice, Inc., you will contribute to creating a supportive environment for patients and their loved ones. Your responsibilities will include:
- Providing patient and family counseling and education
- Participating in comprehensive assessments and developing individualized plans
- Coordinating with team members to ensure seamless care delivery
- Maintaining accurate records and reports
Required Skills and Qualifications:
- Master's degree in Social Work (MSW) from an accredited institution
- Licensed by the State of New York
- At least two years of experience in healthcare or family therapy
- Excellent communication and interpersonal skills
