Overview:

VNS Health is seeking an experienced After Hours Patient Care Specialist to join our team. As a key member of our hospice team, you will be responsible for coordinating patient care and emergent scheduling issues in a fast-paced environment.

Responsibilities:

  • Provide exemplary customer service and support to patients, families, and clinicians
  • Ensure compliance with regulatory requirements and hospice policies
  • Manage non-clinical aspects of the hospice team, including coordination of patient needs and resources
  • Collaborate with the Intake team, IDG team, and other stakeholders to ensure seamless patient care
  • Participate in quality improvement initiatives and contribute to the development of workflows and job processes

Requirements:

  • Associate's degree preferred
  • A minimum of three years of healthcare experience, with one year of hospice-specific experience required
  • Strong knowledge base in hospice regulations and practices
  • Exceptional customer service skills, including phone manner and relationship-building
  • Ability to prioritize multiple tasks and handle simultaneous situations in a professional and calm manner

Estimated Salary: $41,535 - $54,266 per year
