Overview

Hospice of New York is seeking a skilled and compassionate Registered Nurse to join our team as a Case Manager. This dynamic role requires exceptional communication skills, empathy, and the ability to work collaboratively with patients, families, and healthcare professionals.

Key Responsibilities:
  1. Conduct thorough patient assessments to identify physical, emotional, and spiritual needs.
  2. Develop and implement personalized care plans that address the unique requirements of each patient.
  3. Collaborate with interdisciplinary teams to ensure seamless coordination of care.
  4. Provide education and support to patients and their families on pain management, symptom control, and end-of-life care.
  5. Maintain accurate records and documentation to ensure continuity of care.
Requirements:
  • Current RN licensure in New York State.
  • Bachelor's degree in nursing or equivalent experience.
  • Minimum 1 year of experience in hospice or home health care.
  • Strong communication and interpersonal skills.
