Senior Hospice Nurse Specialist

Hospice of New York seeks a compassionate and skilled Registered Nurse to join our team as a RN Case Manager. This dynamic role involves providing continuous nursing assessment, planning, and coordination of care to maximize the comfort of patients with a life-limiting illness.
The ideal candidate will have a strong commitment to the Hospice philosophy of care and excellent communication skills to collaborate with other members of the Interdisciplinary Team. With a focus on delivering high-quality patient-centered care, this role is an excellent opportunity for a dedicated nurse to make a meaningful difference in the lives of patients and their families.
Responsibilities- Conduct initial and ongoing assessments to identify physical, psychosocial, spiritual, and bereavement needs of patients and families.
- Develop and update individualized Plans of Care to reflect changing patient conditions and needs.
- Manage and deliver patient/family care in accordance with the Hospice Plan of Care.
- Maintain accurate records of patient interactions and changes in condition or care.
- Collaborate with healthcare professionals to ensure seamless coordination of care and resource utilization.
- Current RN licensure in New York State.
- Bachelor's degree in Nursing from an accredited program or equivalent experience.
- Knowledge of computer operations and clinical record-keeping preferred.
