We are seeking a skilled and compassionate Registered Nurse to join our Intermountain Homecare & Hospice team as a Hospice Nurse - Patient-Centered Home Care Specialist. This is an exciting opportunity to make a meaningful difference in the lives of patients and their families.

As a Hospice Nurse with our team, you will have the opportunity to develop your clinical skills while delivering compassionate care to individuals in need.
SalaryThe estimated salary for this position is $34.19-$50.73 per hour, based on experience.
Job DescriptionAs a Hospice Nurse with our team, you will be responsible for providing comprehensive, patient-centered care to individuals in their homes. This includes:
- Assessing patients' medical needs and developing individualized care plans
- Providing wound care, IV therapy, and other treatments as needed
- Educating patients and caregivers on treatment plans, medication administration, and health management strategies
- Maintaining accurate and detailed records of patient care
You will work autonomously in the home setting, receiving support from a collaborative healthcare team. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
RequirementsTo be successful in this role, you must have:
- A current RN license in the state of practice
- Current BLS Certification for healthcare providers
- Current drivers license, insured and reliable transportation, and an acceptable driving record
- Ability to communicate effectively both verbally and in writing
- Ability to transfer patients independently
PREFERRED QUALIFICATIONS:
- Bachelor's Degree in Nursing (BSN)
- Two years working as an RN
- Previous experience in an Acute Care, Home Health, or Hospice setting
