Salary: $31-$38 per hour (commensurate with education, experience, licensing, and certifications)

About the Job:

We are seeking a part-time Social Worker (LMSW) to join our team in Orange County. As a member of our interdisciplinary team, you will contribute to patient and family counseling and education, participate in ongoing comprehensive assessments, and assist patients and their families in utilizing community resources.

Responsibilities:

  • Conduct thorough psychosocial assessments of patients and families
  • Develop and implement individualized plans of care that address the unique needs of each patient and family
  • Collaborate with other healthcare professionals to ensure seamless delivery of care
  • Provide emotional support and guidance to patients and their families
  • Maintain accurate and timely records of patient interactions and care plans
  • Participate in bereavement programs and provide support to grieving families

Requirements:

  • Master's degree in Social Work (MSW) from an accredited program
  • Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) certification
  • Minimum two years of supervised experience in healthcare or social services
  • Ability to work effectively with diverse populations and communicate professionally
  • Daily access to a reliable vehicle, valid driver's license, and insurance coverage
