Job Description

As a Hospice Care Specialist with MJHS, you will play a vital role in providing compassionate and high-quality care to patients with life-limiting conditions.

Reporting to the RN Team Manager, your primary responsibilities will include:

  • Triage all calls for Hospice patients from 5pm-8am, providing prompt and professional support when needed.
  • Provide direct nursing care to patients who require home visits, tailoring your approach to meet their unique needs.
  • Develop and update individualized plans of care as required, collaborating closely with physicians to ensure seamless transitions.
  • Document all nursing interventions and patient updates accurately, maintaining precise records within our electronic system.

You will work closely with the primary RN case manager to communicate any changes in patient condition, fostering a collaborative team environment.

This Office Field position offers an attractive salary of $55 per hour, making it an excellent opportunity for experienced nurses looking to make a meaningful difference in the lives of others.

To succeed in this role, you will possess:

  • A graduate degree from an accredited school of Nursing.
  • Minimum 1-2 years of experience in medical/surgical, Community Health, and/or Hospice/oncology Nursing, with a strong foundation in Word, Windows, and Excel.
  • Excellent communication, listening, and teaching skills, enabling you to effectively engage with patients, families, and healthcare professionals.
  • A valid driver's license and reliable vehicle, ensuring timely and efficient responses to patient needs.
