AltaStaff, LLC is a staffing agency that specializes in placing skilled professionals in healthcare positions. As a UAS RN Care Coordinator, you will have the opportunity to work with Registered Nurses to complete Clinical Assessments for New York State Residents.

We are currently hiring for several counties throughout NYS and are offering a competitive hourly rate of $55.50. In addition to a salary, we also provide health, dental, vision, and PTO benefits.

The Job Description:

Job Summary

You will be conducting in-field assessments as part of our team. Your ability to review health status questions, make evaluations, and determinations based on UAS criteria for enrollment will be crucial in this role. You should also be able to work effectively with technology, possess excellent organizational and communication skills, and be able to perform comfortably in a fast-paced environment.

Job Responsibilities
  1. Perform initial in-home pre-evaluations of patients, evaluating their health status, strengths, care needs, and preferences, and guiding the development of individualized long-term care service plans.
  2. Review consumer medical documentation and health referral forms, as relevant to the case.
  3. Enter evaluation data into electronic evaluation forms and transmit, as required.
  4. Document any concerns, conflicting information, or other issues that surface during the evaluation process.
  5. Emphasize continuity of care, reducing or eliminating fragmentation, duplication, and gaps in treatment plans.
  6. Discuss health care options, supports needed, service vendor options, and waiver options with consumers.
  7. Initiate communication between the Call Center and eligible consumers for enrollment into managed care health plans.
Required Licensure and Experience
  • Current RN licensure in New York State.
  • Current active NY driver's license or NY state ID.
  • At least 2 years of experience as an RN in home care.
