**Job Summary:** Tandym Group is seeking a skilled UAS RN to join our team in Manhattan. This role involves conducting UAS assessments in a fast-paced environment and working closely with clients to ensure they receive the best possible care.
**Key Responsibilities:**
- Conduct home visits to assess client needs and develop individual plans of care.
- Determine eligibility of enrollees and ensure that DOH UAS information is complete and accurate.
- Develop and implement effective care plans that meet client needs and preferences.
**Requirements:**
- NYS Registered Nurse (RN) license
- Prior UAS, MLTC or Home Care experience
**What We Offer:**
