Care Coordination Nurse Specialist

Job SummaryThe Care Coordination Nurse Specialist is the first point of contact for potential members and their families from the MLTCP programs. This role conducts the UAS assessment to determine the level of care needed to support applicants/members in their own homes. The nurse specialist completes the UAS assessment tool and makes the determination if an applicant is appropriate for the MetroPlusHealth MLTC program.
This role requires meeting with applicants, family, and caregivers to determine if they meet the clinical qualifications for enrollment. The nurse specialist must also conduct the UAS tool for existing members, score the Attendant Care worker tasking tool, and share potential hours and daycare services with applicants and their families.
Key Responsibilities:- Conducts the UAS assessment for applicants and existing members to determine the level of care needed.
- Meets with applicants, family, and caregivers to discuss their needs and provide guidance on the MLTCP program.
- Completes the UAS assessment tool and scores the Attendant Care worker tasking tool.
- Shares potential hours and daycare services with applicants and their families.
- Reviews findings of assessments and interviews with applicants and their families with the Care Manager.
- Bachelor's Degree required.
- A minimum of two (2) to three (3) years of clinical experience in Certified Home Health Agency (CHHA), Lombardi program, and MLTC experience.
- Registered Nurse license currently active in New York State.
- UAS Certified RN required.
