We are seeking a skilled Field Care Coordinator to join our team at UnitedHealthcare. This is a full-time opportunity that requires working Monday through Friday, 8:30 am to 5:00 pm EST.

The successful candidate will work in the field and coordinate the long-term care needs for patients in the local community. As an essential element of an Integrated Care Model, you will relay pertinent information about member needs and advocate on their behalf.

This role involves assessing, planning, and implementing care strategies tailored to individual patient needs. You will utilize the NYS UAS Assessment as needed to develop a person-centered service plan that meets members' requirements.

Your responsibilities will include identifying and initiating referrals for social service programs, such as financial, psychosocial, community, and state supportive services. You will also manage the care plan throughout the continuum of care as a single point of contact.

As a Field Care Coordinator, you will communicate with all stakeholders to ensure quality coordinated care and services are provided expeditiously to all members. Your advocacy skills will be crucial in representing patients' needs and choices effectively.

This position comes with a competitive salary range of $28.03 to $54.95 per hour, based on factors like education, work experience, certifications, and location.
