Job Description:

Northwell Health is seeking a highly skilled Clinical Assessment RN Manager to join our team. As a key member of our care coordination team, you will be responsible for managing participant onboarding and reassessment processes.

Your primary focus will be on ensuring accurate completion of the Uniform Assessment System of New York (UAS-NY) and facilitating effective collaboration with care delivery teams.

Key Responsibilities:

  • Lead and manage a team of care coordinators to ensure efficient participant onboarding and reassessment processes
  • Develop and implement quality improvement initiatives to enhance participant outcomes and process efficiency
  • Collaborate with care delivery teams to ensure seamless transitions and optimal participant care
  • Monitor and analyze data to identify areas for improvement and develop targeted solutions

Requirements:

  • Bachelor of Science in Nursing (BSN) or higher
  • Minimum 3 years of experience in managed care programs or similar settings
  • Proven leadership and collaboration skills
  • Excellent communication and interpersonal skills

Salary: The estimated salary range for this position is $140,000 - $170,000 per year, based on the location and industry standards.
