Long-Term Care Coordinator

We're looking for a dedicated Long-Term Care Coordinator to join our team at 1st Choice, LLC. In this role, you'll conduct in-home and remote assessments for individuals seeking community-based long-term care, utilizing your expertise to empower them to live independently.
As a key member of our Independent Assessor (IA) Project, you'll work closely with interdisciplinary teams to connect consumers with community resources, making a tangible impact on the lives of New York's most vulnerable populations.
We offer a competitive salary range of $65,000 - $80,000 per year, depending on experience and qualifications.
Key Responsibilities:- Conduct assessments of individuals' health, functional abilities, strengths, and care needs.
- Review medical records and referrals to provide informed evaluations.
- Collaborate with interdisciplinary teams to connect consumers with community resources.
Requirements:
- Graduate of an accredited nursing program (Bachelor's degree preferred).
- Active RN license in New York State.
- Familiarity with UAS assessments and experience working with geriatric or chronically ill populations is preferred.
