We are seeking a highly skilled Medical Care Coordinator to join our team in New York City.

Job Summary:

The Medical Care Coordinator will be responsible for conducting in-home pre-evaluations of patients, reviewing medical documentation, and entering evaluation data into electronic forms.

Key Responsibilities:

  • Perform initial in-home pre-evaluations of patients, evaluating health status, strengths, care needs, and preferences.
  • Review consumer medical documentation and health referral forms as relevant to the case.
  • Enter evaluation data into electronic evaluation forms and transmit as required.
  • Document any concerns, conflicting information, or other issues that surface during the evaluation process.
  • Emphasize continuity of care, reducing fragmentation, duplication, and gaps in treatment plans.
  • Discuss health care options, supports needed, service vendor options, and waiver options with consumers.
  • Initiate communication between the Call Center and eligible consumers for enrollment into managed care health plans.
  • Enhance communication and collaborative relationships with interdisciplinary care team members to improve care coordination and facilitate service delivery.
  • Collect quality review data and required documentation to support outcome measurements and record case notes into CRM.
  • Identify opportunities for health promotion and illness prevention.
  • Maintain a comprehensive working knowledge of community resources, payor requirements, and network services for the target population.
  • Meets all standards established for this position as outlined in the corresponding annual performance criteria and bonus template.
  • Performs other duties as assigned by the Regional Nurse Manager or other project management.

Requirements:

  • Two years of experience in a HOME setting within the last ten years, including working in Certified Home Health Agencies (CHHA), Licensed Home Care Service Agencies (LHCSA), or Managed Long-Term Care (MLTC).
  • Conducting assessments similar to the UAS, such as OASIS.
  • Providing primary care to homebound persons.
  • Care management for persons receiving home and community-based services, including Hospice and Assisted Living.
  • Home care experience as a paraprofessional or Licensed Practical Nurse caring for a minimum of 2 patients per week.
  • A strong ability to work independently and effectively with multi-generational consumers.

Preferred Qualifications:

  • Experience conducting assessments in UAS-NY, especially in the last 365 days.
  • Experience caring for a geriatric population and managing their chronic diseases, especially in the home setting.
  • A combination of home care experience as a paraprofessional or LPN and RN caring for a minimum of 2 patients per week.

Salary: $65,000 - $80,000 per year, depending on experience.
