Role Overview:
The Substance Use Support Specialist plays a crucial role in guiding patients and their families through the complexities of the healthcare system. By leveraging both personal interaction and advanced technology, this position aims to enhance the quality of care while ensuring it remains accessible and affordable for all.


Key Responsibilities:
As a Substance Use Support Specialist, you will:

  • Actively engage with patients to assess their readiness for treatment and connect them with appropriate resources.
  • Utilize motivational interviewing techniques to foster respectful and culturally sensitive communication.
  • Stay informed about various substances, withdrawal symptoms, and treatment options to effectively educate and support patients.
  • Serve as a liaison between patients and addiction medicine clinics, facilitating their journey towards recovery.
  • Promote the expansion of medication-assisted treatment (MAT) programs within the healthcare setting.
  • Coordinate transitions of care by managing referrals, appointments, and other logistical needs.
  • Maintain regular communication with the care team to enhance service delivery and patient outcomes.
  • Develop relationships with community resources to assist patients in achieving overall wellness.
  • Document all patient interactions accurately and in compliance with program standards.


Qualifications:
The ideal candidate will possess:

  • A high school diploma or GED (preferred).
  • A commitment to patient confidentiality and a nonjudgmental approach.
  • Understanding of substance use disorders and medication-assisted treatment.
  • Healthcare experience and proficiency in medical terminology.
  • Strong computer skills, including familiarity with Google Suite and Excel.
