Centinela Hospital Medical Center is seeking a highly skilled Substance Use Navigator to join our team. As a key member of our healthcare team, you will play a vital role in helping patients access evidence-based treatment for substance use disorders.
Key Responsibilities- Conduct outreach and identification of patients with substance use disorders
- Conduct initial brief assessments and introduce patients to medication-assisted treatment (MAT) programs and services
- Facilitate appointments at outpatient MAT clinics and serve as a resource and coach for patients
- Advocate for culture change in the hospital and provide emotional support, crisis intervention, and counseling for patients
- Assist with providing case management and milieu management, and complete psychosocial assessments
- Coordinate discharge planning and complete necessary documentation
- Master's Degree in Social Work or related field
- Minimum one year hospital or outpatient experience with emphasis in mental health care
- Experience working with a diverse population with a wide range of problems
- Nonjudgmental, energetic, and positive approach to assisting patients with substance use disorders
- Understanding of substance use disorder as a medical condition and the role of MAT in treatment and harm reduction
- Current BCLS (AHA) certificate
