Position Overview:
The primary role of the Youth Forensic Services Coordinator is to connect minors engaged in the Juvenile Justice system with essential behavioral health resources.

This position requires a strong collaborative effort with both the Juvenile Court and probation departments.

Key Responsibilities:
This individual will deliver tailored Service Coordination services that align with the specific needs of the child or family.

Core responsibilities include:

  • Developing a comprehensive service coordination strategy that identifies personal and functional objectives aimed at enhancing individual wellness and minimizing involvement in the Juvenile Justice System.
  • Making appropriate referrals to behavioral health, physical health, educational services, and additional support systems as determined by thorough assessments.
  • Monitoring all services and objectives closely, ensuring ongoing care coordination and persistent family engagement.
  • Acting as the primary contact for families and involved systems and providers.

Duties and Responsibilities:
Implement and provide services in accordance with the Single Point of Accountability responsibilities:

  • Serve as the primary resource for clients and their families.
  • Ensure effective safety net resources are available for clients.
  • Clearly communicate expectations to clients and their guardians regarding the system's operations.
  • Conduct periodic assessments and facilitate cross-system planning to meet client needs while leveraging their strengths.
  • Prepare and lead service planning meetings, providing follow-up as necessary.
  • Ensure coordination of services across systems and that services are being delivered effectively.
  • Provide culturally competent services, considering race, religion, gender, sexual orientation, age, and ethnicity.
  • Assist clients in developing and utilizing natural support systems.
  • Advocate persistently for those served and provide feedback on systemic issues.
  • Maintain a positive outlook that fosters recovery and community inclusion.

Program Compliance:
Maintain adherence to Juvenile Forensic utilization requirements and program standards on a monthly basis.

Engage with probation officers to discuss suitable mental health services for youth, offering technical assistance and acting as a liaison between behavioral health providers and the court system.

Provide support and technical assistance to regional juvenile probation offices and local magistrates.

Attend interagency meetings and court hearings, offering advocacy and support to individuals and families.

Caseload Management:
Effectively manage a caseload by:

  • Facilitating interagency meetings.
  • Maintaining regular contact with families and service providers.
  • Providing ancillary services as needed.
  • Attending necessary meetings and court hearings to support families.

Ensure compliance with HIPAA confidentiality policies.

Collaboration:
Coordinate with families, service providers, Children Youth and Families, Juvenile Probation, educational institutions, and community resources.

Participate in all relevant meetings and systems (court hearings, permanency planning, IEPs, etc.).

Provide educational advocacy by attending school meetings and offering technical assistance.

Qualifications:
Essential skills include:

  • Ability to work collaboratively in a team-oriented environment.
  • Attention to detail and strong organizational skills.
  • Self-starter with creativity in problem-solving.
  • Proficiency in Microsoft Office applications.
  • Adaptability to new software and diverse agency interactions.

Requirements:
Possess necessary clearances and a valid motor vehicle license with access to a personal vehicle. Maintain current auto insurance meeting minimum coverage requirements.

Education:
A Bachelor's Degree in relevant fields such as Social Work, Psychology, Criminal Justice, Counseling, or related social sciences from an accredited institution is required.
