The primary role of the Youth Forensic Services Coordinator is to connect children and teenagers engaged in the Juvenile Justice system with appropriate behavioral health resources. This position requires extensive collaboration with both the Juvenile Court and probation departments. The individual will deliver Service Coordination tailored to the specific needs of the child or family involved.
Key Responsibilities:
- Develop and implement a detailed service coordination strategy aimed at identifying personal and functional objectives that enhance individual wellness and minimize involvement in the Juvenile Justice System.
- Facilitate referrals to necessary behavioral health services, physical health care, educational support, and additional resources based on thorough assessments.
- Maintain close monitoring of all services and objectives, ensuring ongoing care coordination and consistent family engagement.
- Act as the primary contact for families and the various systems and providers involved in the case.
Specific Duties Include:
- Executing services in accordance with the Single Point of Accountability responsibilities, ensuring effective communication and resource availability for clients and their families.
- Conducting periodic assessments and collaborative planning to address the client's needs while leveraging their strengths.
- Organizing and leading service planning meetings, providing follow-up support as necessary.
- Ensuring cross-system coordination of services and their delivery.
- Delivering culturally sensitive services that respect diverse backgrounds.
- Encouraging clients to develop and utilize natural support systems.
- Advocating persistently for clients and providing feedback on systemic challenges.
- Maintaining a positive outlook that fosters recovery and community inclusion.
Program Compliance:
- Adhere to Juvenile Forensic utilization requirements and program standards on a monthly basis.
- Collaborate with probation officers to assess the mental health service needs of children and adolescents.
- Provide technical assistance to regional juvenile probation offices and local magistrates.
- Participate in interagency meetings and court hearings, offering advocacy and support to families.
- Effectively manage a caseload, facilitating interagency meetings and maintaining communication with families and service providers.
- Attend court hearings as needed to support families navigating the judicial process.
- Comply with HIPAA confidentiality regulations.
- Coordinate with families, service providers, and community resources, attending all relevant meetings.
- Advocate for educational needs by attending school meetings and providing necessary support.
Required Skills and Knowledge:
- Ability to thrive in a collaborative team environment.
- Strong organizational skills and attention to detail.
- Proficiency in Microsoft Office Suite and adaptability to new software.
- Capacity to work with diverse agencies and communities.
Essential Qualifications:
- Possession of required clearances.
- Valid driver's license and access to a vehicle.
- Knowledge of Service Coordination guidelines and regulations.
- Excellent oral and written communication skills.
- Ability to work independently and manage time effectively.
- Full-time commitment of 38.75 hours per week.
Educational Background:
A Bachelor's Degree in relevant fields such as Social Work, Psychology, Criminal Justice, Counseling, or related social sciences from an accredited institution is required. Candidates may also be eligible for the BH Fellows Program, which offers student loan repayment opportunities for individuals working in the public behavioral health system.
