Job Summary

We are seeking a skilled Social Worker to join our team at Christus Health. As a Social Worker, you will play a critical role in ensuring the well-being of our patients and their families.

Key Responsibilities
  • Resource Management

Effectively manage resources to ensure efficient use and minimize waste.

Scheduling

Schedule personal time off to ensure adequate availability for necessary services, including Utilization Review, Discharge Planning, and Social Services.

Training and Education

Assist in the orientation, training, and education of department members to ensure they can perform their job duties effectively.

Equipment Use

Use computer, copy/fax machine, and other office equipment in a safe and appropriate manner.

File Maintenance

Assist in maintaining files and documents as needed to meet hospital needs and requirements of regulating agencies, such as CMS, NMDOH, and DNV.

Interviews and Assessments

Interview patients and their families, physicians, and nursing staff to obtain information relevant to medical problems and discharge planning needs.

Liaison Services

Act as a liaison between the hospitalized patient, their family, their physician, and community health agencies, facilities, and allied services that will provide continued care after discharge from the hospital.

Counseling Services

Provide brief counseling services to patients and their families for problems related to hospitalization and future care needs.

Community Resource Management

Maintain current information on all frequently used community resources, financial benefits programs, and placement facilities, updating departmental files and handouts for patients.

Discharge Planning

Assist patients and their families in formulating and implementing appropriate plans for post-discharge health care.

Nursing Home Placement

Assist with nursing home placement inquiries and documentation required for placement problems.

Policies and Procedures

Develop policies and procedures as needed for specific Social Work needs.

Requirements
  • Education

Master's degree in Social Work (MSW) is required.

Licenses and Certifications

Licenses and certifications, such as L-LicMastersSW, are required. Heartsaver and BLS certifications are preferred.
