POSITION SUMMARY: Under the guidance of a licensed supervisor, the Care Coordinator II - Delegated - Licensed collaborates closely with patients and their families, as well as the Interdisciplinary Care Team. This role is essential in proactively assessing, planning, implementing, monitoring, and evaluating options and services to create a tailored care plan for each patient throughout their healthcare journey. The process encompasses the patient's various care environments and healthcare system requirements. A strong emphasis is placed on customer service, goal-setting knowledge, and outcome measurement to ensure optimal patient results in a cost-effective manner. Compliance with procedures and timelines as outlined in the Managed Care Organization (MCO) contract and PMS protocols is mandatory.
Qualifications
REQUIREMENTS
EDUCATION AND/OR EXPERIENCE:
- Graduation from an accredited nursing program (LPN or RN) with five years of relevant healthcare experience and a valid nursing license; OR
- A Bachelor's or Master's degree in social work, nursing, or another healthcare discipline, along with a nursing or non-independent behavioral health license in NM (e.g., LPN, RN, LBSW, LPC, LMHC, LMSW, LAMFT) and one year of healthcare experience.
- Proficient in personal computer usage and software for word processing, with the ability to extract data from relevant systems.
- Self-motivated with strong verbal and written communication capabilities.
- Highly organized with exceptional follow-through abilities.
- Adaptable to change and committed to quality improvement initiatives.
- Engagement in ongoing education, certification, and self-development to stay current with industry standards and objectives related to care coordination.
- Experience with Electronic Health Records.
- Familiarity with Motivational Interviewing techniques.
- Knowledge of New Mexico and local community resources.
- Must possess and maintain a valid nursing license or non-independent behavioral health license in NM (e.g., LPN, RN, LBSW, LPC, LMHC, LMSW, LAMFT) at the time of hire.
- Ability to obtain First Aid and BLS/CPR certifications within 30 days of hire, with the responsibility to keep certifications current.
- Certified Case Manager (CCM) designation is preferred.
This position necessitates successful completion of a criminal records check post-offer of employment.
- Compliance with required testing and immunizations as per PMS Human Resources Policy.
- May require personal transportation for job duties.
- Must hold and maintain a valid Driver's License.
- Must provide a current motor vehicle record and maintain personal automobile insurance throughout employment.
- Must submit to a motor vehicle record check as needed and maintain a satisfactory driving record.
