ROLE SUMMARY: The Senior Care Coordinator collaborates closely with patients and their families, as well as the Interdisciplinary Care Team, to proactively evaluate, plan, implement, monitor, and assess various options and services. This role aims to create a tailored, comprehensive care plan for patients throughout their healthcare journey. The focus is on addressing the diverse environments of care and the intricate needs of the healthcare system. Strong customer service skills, goal-setting knowledge, and outcome measurement are essential to ensure optimal patient results in a cost-effective manner. Compliance with procedures and timelines as outlined in the Managed Care Organization (MCO) contract and PMS protocols is required.

Qualifications

ESSENTIAL REQUIREMENTS:
  • Bachelor's or Master's degree in social work, nursing, or a related healthcare field; AND a minimum of two years of relevant healthcare experience.
  • Proficient in personal computer usage and software for document processing, with the ability to extract data from pertinent systems.
  • Self-motivated individual with strong verbal and written communication abilities.
  • Highly organized with exceptional follow-through capabilities.
  • Adaptable to change and committed to quality improvement initiatives.
  • Engagement in ongoing education, certification, and self-development to stay aligned with industry standards and objectives in care coordination.
PREFERRED QUALIFICATIONS:
  • Experience with Electronic Health Records.
  • Familiarity with Motivational Interviewing techniques.
  • Knowledge of local community resources in New Mexico.
  • Bilingual proficiency (English/Spanish or English/Navajo).
CERTIFICATIONS AND LICENSES:
  • Must obtain and maintain the following by the start date:
  • Nursing license in New Mexico (e.g., LPN or RN); OR
  • Independent behavioral health license as a Mental Health professional in New Mexico (e.g., LCSW, LISW, LPCC, LMFT, LCP, or LPAT).
  • First Aid and BLS/CPR certifications must be acquired within 30 days of hire, with a responsibility to keep certifications current.
  • Certified Case Manager (CCM) certification is preferred.
SCREENING REQUIREMENTS:

This position mandates successful completion of an initial post-offer employment screening:
  • Compliance with PMS Human Resources Policy regarding healthcare personnel testing and immunization requirements, including TB, Hepatitis B (consent or waiver), Tdap, MMR, and Varicella.
  • Criminal Records Check.
DRIVING REQUIREMENTS:
  • May require personal transportation for job duties.
  • Must possess and maintain a valid Driver's License.
  • Must provide a current motor vehicle record and maintain personal automobile insurance throughout employment.
  • Must submit to a motor vehicle record check as needed and maintain a satisfactory driving record.
  • Compliance with PMS Policies on Automobile Usage and Drug-Free Workplace is required.
  • May be required to complete transportation-related training.
