Position Overview
This role necessitates an initial in-person probationary period, after which a hybrid-remote schedule may be considered. The Care Coordinator II operates under licensed supervision, collaborating closely with patients, families, and the Interdisciplinary Care Team to proactively assess, plan, implement, monitor, and evaluate options and services tailored to the individual needs of patients throughout their care journey.

Key Responsibilities
- Work in partnership with patients and their families to create and execute a personalized care plan.
- Address the multifaceted environments of care and healthcare system requirements for each patient.
- Focus on exceptional customer service, goal setting, and outcome measurement to ensure optimal patient results in a cost-effective manner.
- Adhere to established procedures and timelines as per the Managed Care Organization (MCO) contract and organizational protocols.

Qualifications
Education and Experience:
- Graduation from an accredited nursing program (LPN or RN) with five years of relevant healthcare experience, or a Bachelor's or Master's degree in social work, nursing, or a related healthcare field, along with a nursing or non-independent behavioral health license in New Mexico and one year of healthcare experience.
- Proficiency in personal computer usage and software for documentation and data retrieval.
- Strong verbal and written communication skills, self-motivated, and highly organized with a commitment to quality improvement.
- Ongoing education and certification are required to stay current with industry standards related to care coordination.

Preferred Qualifications
- Experience with Electronic Health Records and Motivational Interviewing.
- Familiarity with New Mexico and local community resources.

Licenses and Certifications
- Must possess and maintain a valid nursing license or non-independent behavioral health license in New Mexico.
- Ability to obtain First Aid and BLS/CPR certifications within a specified timeframe upon hire.
- Certifications must align with the requirements outlined in the organizational training resources.

Screening Requirements
- Successful completion of a criminal records check is required.
- Compliance with healthcare personnel testing and immunization requirements is mandatory.

Driving Requirements
- May require personal transportation for job duties. A valid Driver's License and current motor vehicle record are necessary.
- Must maintain personal automobile insurance throughout employment.
