Role Summary:
Under the guidance of a licensed supervisor, the Care Coordinator II is responsible for:
- Collaborating closely with patients and their families, as well as the Interdisciplinary Care Team, to proactively assess, plan, implement, monitor, and evaluate services tailored to each patient's unique needs.
The position emphasizes exceptional customer service skills, knowledge of goal-setting methodologies, and outcome measurement to ensure optimal patient results in a cost-effective manner.
Compliance with procedures and timelines as outlined in the Managed Care Organization (MCO) contract and PMS protocols is essential.Qualifications
Education and Experience Requirements:
Graduation from an accredited nursing program (LPN or RN) with a minimum of five years of relevant healthcare experience, along with a valid nursing license; OR a Bachelor's or Master's degree in social work, nursing, or another healthcare discipline, coupled with a nursing or non-independent behavioral health license in NM (e.g., LPN, RN, LBSW, LPC, LMHC, LMSW, LAMFT) and at least one year of healthcare experience.
Proficiency in personal computer applications and software for word processing, as well as the ability to extract data from relevant systems, is required.
The ideal candidate will be a self-starter with strong verbal and written communication abilities, highly organized, and adept at following through on tasks. Comfort with change and quality improvement processes is crucial.Continuous education, certification, and self-development to stay current with industry standards and objectives related to care coordination are expected.
Preferred Qualifications:Experience with Electronic Health Records, familiarity with Motivational Interviewing techniques, and knowledge of local community resources in New Mexico are advantageous.
Licenses and Certifications:
Must possess and maintain a valid nursing license or non-independent behavioral health license in NM (e.g., LPN, RN, LBSW, LPC, LMHC, LMSW, LAMFT) at the time of hire. First Aid and BLS/CPR certifications must be obtained within 30 days of hire.
It is the employee's responsibility to keep certifications current at all times. Certification as a Certified Case Manager (CCM) is preferred.
Screening Requirements:
This position mandates successful completion of a criminal records check post-offer of employment. Compliance with required testing and immunizations is necessary as per PMS Human Resources Policy.
Driving Requirements:
May necessitate the use of personal transportation for job duties. A valid Driver's License is required, along with a current motor vehicle record. Personal automobile insurance must be maintained throughout employment.
Employees must comply with PMS Policies on Automobile Usage and maintain a satisfactory driving record.
