Job Summary

The Program Coordinator is a key member of our team, responsible for overseeing the delivery of services in designated homes. This role requires strong leadership and communication skills, as well as the ability to work effectively with staff, individuals served, and their families.

Key Responsibilities
  • Supervise staff and schedule them for assigned shifts
  • Take disciplinary action as necessary
  • Function as a Program Coordinator for individuals served
  • Assist in developing, preparing, and monitoring individuals served plans of care
  • Interview prospective individuals served and families
  • Coordinate and arrange appointments, meetings, and other activities
  • Serve as a support for individuals served, their families, and Evergreen staff
  • Monitor budgetary requirements and expenditures
  • Ensure compliance with policies, procedures, regulations, and guidelines
  • Assist in scheduling individuals served training and maintaining documentation
  • Work flexible hours as requested by Executive Director
  • Staff performance evaluations
  • Participate actively in hiring staff in their perspective home assignment
Requirements
  • Prefer a four (4) year degree or any combination of education and experience in serving individuals with disabilities
  • At least two (2) years' experience in serving individuals with disabilities
  • Experience in supervising staff, monitoring budgets, and following licensing and regulatory guidelines
  • Clear understanding of administrative procedures and personnel management
  • Understanding of Medicaid guidelines as they relate to programming
  • Understanding of guidelines for individuals served receiving Supported Living, Assisted Living, or In-Home Waiver Services
  • Working knowledge of Windows 7, Office 2010 applications, and aptitude to learn other software programs as required for this position
Working Environment

This role requires the ability to work in a variety of settings and environments, both indoors and outdoors. The successful candidate will be able to maintain good working relationships with staff, professionals, and individuals served, and will be able to work effectively in a fast-paced environment with multiple task deadlines.
