About the Role

The Mental Health Association in Ulster County, Inc. is seeking a full-time (37.5 hours per week) Residential Coordinator for our Supportive Apartment Program. This role involves delivering case management services by developing and executing personalized rehabilitation and service plans tailored for adults experiencing mental health challenges and associated substance use issues. The focus is on providing individualized, client-centered support to improve the functioning and quality of life for our clients. The Coordinator will engage with individuals living independently in the community, conducting regular visits throughout the week.

The standard work schedule is Monday through Friday, from 9 AM to 5 PM.

The starting hourly wage for this position is $19.20.

Key Responsibilities

  • Develop and maintain individual service plans, conduct service plan evaluations, and prepare monthly progress documentation.
  • Facilitate support team meetings and manage all necessary admission and discharge documentation.
  • Maintain daily progress records, monitor goal achievements, and oversee medication administration.
  • Ensure clients receive appropriate medical care and connect them with vocational and educational resources in the community.

Required Qualifications

  • Applicants must possess a High School Diploma and demonstrate a strong commitment to supporting adults with significant mental health and substance use challenges.
  • Excellent writing, computer, organizational, and interpersonal skills are essential, along with the ability to prioritize tasks and work autonomously.
  • A clean, valid NYS driver's license with a minimum of three years of driving experience is required.
