Unlimited Care Inc. is seeking a Client Support Coordinator for participants in the Medicaid Waiver Programs, specifically within the Nursing Home Transition and Diversion and Traumatic Brain Injury initiatives.

Key Responsibilities:

• Support Service Coordinators in crafting detailed Service Plans, delivering services, and coordinating medical appointments and transportation needs.

• Manage inquiries via phone, email, and fax, ensuring messages are appropriately directed and service challenges are addressed promptly.

• Organize and maintain all documentation and reports systematically, ensuring timely updates and annual reviews of all files.

• Participate in Team Meetings and service plan development discussions as necessary.

Qualifications:

· An associate's degree is required.

· Proficiency in Microsoft Office Suite is essential.

· A collaborative team player who can also work independently.

· Strong verbal and written communication abilities are necessary.

· Capability to thrive in a dynamic, multi-tasking environment.

· A valid driver's license is required, along with the ability to travel as needed.
