Position Summary
The Mental Health Association in Ulster County, Inc. is seeking a dedicated individual for a full-time role in our Residential Program. The Housing Support Case Manager is responsible for managing a caseload of clients who live independently in their own residences while receiving rental assistance. The primary focus of this role is to promote long-term housing stability and maintenance.
Key Responsibilities
- Collaborate with the Residential Director and Site Supervisor to achieve departmental objectives, ensuring confidentiality for all program participants.
- Assist clients in creating and advancing their personalized goal plans.
- Maintain regular contact with housing clients based on their unique needs and circumstances.
- Facilitate referrals to essential services, including healthcare, mental health support, substance abuse treatment, and legal assistance, while guiding clients through these processes.
- Accurately document all client interactions and maintain comprehensive records.
Qualifications
- A High School Diploma is required, along with at least one year of experience in Human Services.
- Strong writing, organizational, and interpersonal skills are essential, with the ability to prioritize tasks and work autonomously.
- A valid NYS driver's license with a clean driving record is mandatory.
