Position Summary:

The Mid-Hudson permanency resource center specializes in delivering tailored services to families involved in post-adoption, post-guardianship, and kinship caregiving across various counties. The Case Manager is instrumental in community outreach, conducting in-person visits with families, and maintaining accurate case documentation. This full-time role offers flexibility within a collaborative team setting.

Key Responsibilities:

  • Oversee a caseload of families to ensure adherence to program performance objectives.
  • Conduct in-person visits with families, documenting evaluations and service strategies.
  • Facilitate counseling sessions that incorporate trauma-informed care and motivational interviewing techniques.
  • Support program initiatives and activities, including parent education, respite care, and family engagement efforts.
  • Assist with community outreach and presentations to local organizations.

Qualifications:

  • Bachelor's Degree in Human Services or a related discipline is preferred.
  • Experience with at-risk families and familiarity with community resources is essential.
  • Excellent interpersonal and communication abilities.
  • Proficient in Microsoft Office and Google Workspace applications.
  • Possession of a valid New York State driver's license and access to a reliable vehicle.
  • Comfortable working with diverse populations while maintaining confidentiality.
  • Successful completion of required pre-employment background checks.
