Position Overview:

The Hospice Social Worker plays a crucial role in delivering emotional and medical social support to patients facing terminal illnesses and their families, ensuring adherence to a comprehensive care plan.

Key Responsibilities:

  • Collaborates with the healthcare team to formulate and execute a care strategy that may involve home-based treatment or transition to a hospice facility. Evaluates incoming patient referrals to determine eligibility for hospice services and provides guidance for alternative agency referrals when necessary.
  • Conducts assessments of patients and families to understand their psychological responses to terminal conditions, offering compassionate counseling and education. Performs home visits and engages with families in various care settings to fulfill essential responsibilities.
  • Analyzes financial circumstances and aids families in navigating available resources. Provides assistance in understanding and utilizing benefits from Medicare, Medicaid, and private insurance plans.
  • Offers support and guidance to families regarding funeral planning and arrangements.
  • Recommends policy and procedural improvements to the executive director concerning social work services and bereavement support.
  • Acts as a resource for professional and community organizations focused on grief and bereavement issues.

Educational Background:

  • A Master's degree in Social Work is mandatory.
  • Must possess a valid license from the state Board of Social Work Examiners.
  • A minimum of one year of experience post-master's degree is required.
  • A valid driver's license is necessary.
  • Commitment to the principles and philosophy of hospice care is essential.

Physical Requirements:

  • Ability to navigate various home environments, including traversing uneven terrain and climbing stairs.
  • Good auditory and verbal communication skills for effective interaction with patients and families.
  • Visual acuity for driving and assessing patients' physical conditions.
