About the Role

Kaiser Permanente is seeking a highly skilled Social Work Case Manager to join our team in Arlington, Virginia.

Key Responsibilities

  • Conduct comprehensive psychosocial assessments to identify patient needs and develop personalized plans of care.
  • Facilitate access to community resources and services, promoting cost-effective and efficient care.
  • Ensure accurate and timely documentation of case management activities, adhering to organizational standards.
  • Analyze patient and program outcomes to identify areas for improvement and optimize case management strategies.
  • Provide supportive counseling and education to patients, families, and caregivers, addressing end-of-life issues and advanced directives.
  • Maintain current knowledge of laws, regulations, and policies related to social work practice, adhering to the NASW Code of Ethics.
  • Perform other related duties as assigned.
Requirements
  • Minimum one year of clinical social work experience in a healthcare setting.
  • Masters degree in social work (MSW) required.
  • Licensed Independent Social Worker (District of Columbia) or Licensed Graduate Social Worker (District of Columbia) within six months of hire.
  • Licensed Master Social Worker (Maryland) or Licensed Master's Social Worker (Virginia) within six months of hire.
Preferred Qualifications
  • Experience with computer software programs in a Windows environment.
  • Knowledge of community systems and resources in the defined service area.
  • Knowledge of regulatory issues for the Mid-Atlantic area.
