We are seeking a skilled and compassionate Nurse Care Specialist to join our team at Remarkable Hospice Companies. As a key member of our care team, you will provide high-quality patient care and support to individuals in their homes.
Key Responsibilities:
- Provide personalized care and support to patients in their homes, including administration of medication, wound care, and other treatments as needed.
- Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
- Conduct thorough assessments of patients' physical and emotional needs, and document observations and interventions in patient records.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and transitions.
- Participate in ongoing education and training to stay current on best practices and industry developments.
Requirements:
- Current Practical/Vocational Nursing License in the state where you intend to work.
- Minimum of 1 year of nursing experience, with a preference for experience in home health or hospice care.
- Strong organizational and communication skills, with the ability to work independently and as part of a team.
- Current CPR certification and proof of current auto insurance.
- Proof of Covid vaccine, medical exemption, or religious exemption.
