Key Responsibilities:

  • Provide personalized care and support to patients in their homes, including administration of medication, wound care, and other treatments as needed.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Conduct thorough assessments of patients' physical and emotional needs, and document observations and interventions in patient records.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and transitions.
  • Participate in ongoing education and training to stay current on best practices and industry developments.

Requirements:

  • Current Practical/Vocational Nursing License in the state where you intend to work.
  • Minimum of 1 year of nursing experience, with a preference for experience in home health or hospice care.
  • Strong organizational and communication skills, with the ability to work independently and as part of a team.
  • Current CPR certification and proof of current auto insurance.
  • Proof of Covid vaccine, medical exemption, or religious exemption.
