Job Summary

Kaiser Permanente is seeking a skilled Social Work Specialist to join our team in Arlington, Virginia.

Key Responsibilities
  • Collaborate with healthcare teams to coordinate patient care and services.
  • Conduct comprehensive psychosocial assessments to identify patient needs and goals.
  • Develop and implement plans of care to promote patient independence and quality of life.
  • Facilitate access to community resources and services to support patient needs.
  • Provide education and support to patients, families, and caregivers on health-related topics.
  • Monitor and analyze patient outcomes to identify areas for improvement.
Requirements
  • Master's degree in social work (MSW) required.
  • Minimum three (3) years of clinical experience and two (2) years in case management required.
  • Licensed Clinical Social Worker (LCSW) certification required within six months of hire.
  • Case Manager Certificate required within thirty-six months of hire.
Preferred Qualifications
  • Experience with computer software programs in a Windows environment.
  • Knowledge of community systems and resources in the defined service area.
  • Knowledge of regulatory issues for the Mid-Atlantic area.
