Kaiser Permanente is seeking a skilled Social Work Specialist to join our team in Arlington, Virginia.

- Conduct comprehensive psychosocial assessments to identify patient needs and goals.
- Develop and implement plans of care to promote patient independence and quality of life.
- Facilitate access to community resources and services to support patient needs.
- Provide education and support to patients, families, and caregivers on health-related topics.
- Monitor and analyze patient outcomes to identify areas for improvement.
- Master's degree in social work (MSW) required.
- Minimum three (3) years of clinical experience and two (2) years in case management required.
- Licensed Clinical Social Worker (LCSW) certification required within six months of hire.
- Case Manager Certificate required within thirty-six months of hire.
- Experience with computer software programs in a Windows environment.
- Knowledge of community systems and resources in the defined service area.
- Knowledge of regulatory issues for the Mid-Atlantic area.
