Position Overview

We are seeking a Licensed Practical Nurse for Continuous Care to become a vital part of our team. This role will report to the Executive Director/Administrator or Director of Clinical Services and will work under the guidance of a Registered Nurse to deliver continuous care visits to patients and families in various settings.

The Licensed Practical Nurse will:

  • Monitor patients closely and communicate any changes in their condition.
  • Facilitate the management of acute medical symptoms during critical periods.
  • Provide patient care services under the supervision of an RN, ensuring a reasonable case load.
  • Assess patient and family needs, contributing to the development and implementation of individualized care plans.
  • Administer medications and monitor vital signs as part of direct patient care.
  • Educate and support patients and their families regarding their healthcare requirements.
  • Document findings and changes in patient conditions, reporting them to the Registered Nurse and interdisciplinary team.
Qualifications

The ideal candidate will possess:

  • A diploma from an accredited Practical Nursing program.
  • A minimum of two years of experience as an LPN/LVN in long-term care, hospice, or home health settings.
  • Preferred experience of three to five years in similar roles.
  • A compassionate and empathetic approach to patient care.
  • The ability to assess and respond effectively to the needs of patients, families, and caregivers.
  • Current CPR certification.
