Job Summary

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Weekend Baylor. The successful candidate will be responsible for providing skilled nursing care to our patients in their homes, working a 4-day weekend shift that includes Saturday and Sunday.

Responsibilities

  • Provide skilled nursing care to patients in their homes, under the direct supervision of a Registered Nurse (RN)
  • Perform selected skilled actions in the provision of curative, rehabilitative, palliative, or preventative nursing care
  • Make decisions based on individual patient needs, nursing experience, and educational preparation
  • Collaborate with the healthcare team to develop and implement individualized care plans
  • Document patient care and progress in a timely and accurate manner

Requirements

  • Current State LPN or LVN License
  • Graduate of an accredited practical nurse or vocational nursing program
  • At least one year of nursing experience, preferably in community/home health or medical/surgical settings
  • Valid BLS certification
  • Good verbal and written communication, and organization skills
  • Licensed driver with an automobile that is in good working order and insured
