Key Responsibilities- Deliver quality nursing care to patients through interpersonal contact and provide care and services to allow patients to attain or maintain the highest practicable physical, mental, and psychosocial well-being.
- Monitor and report changes in the patient's condition, ensuring timely and effective interventions.
- Assign duties, instruct, and provide clinical supervision to Certified Nursing Assistants (CNAs) to ensure seamless care delivery.
- Collaborate with the healthcare team to develop and implement individualized care plans that meet the unique needs of each patient.
- Graduation from an accredited LPN program.
- Current licensure/registration in the applicable State, with a valid Licensed Practical/Vocational Nurse (LPN/LVN) license in good standing.
- One (1) year of skilled nursing experience, preferably in a long-term care setting.
- CPR certification upon hire or obtain during orientation, with ongoing maintenance throughout employment.
