Job Title: Care Coordinator

Job Summary:

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Wisconsin Community Services. As a Care Coordinator, you will play a vital role in providing intensive and individualized care coordination for children and adolescents under the Wraparound Practice Model.

Key Responsibilities:

  • Maintain a caseload of approximately 14 youth and families, providing regular contact and support to ensure their needs are met.
  • Develop and implement comprehensive crisis plans, working closely with families and service providers to ensure a safe and supportive environment.
  • Facilitate Child and Family Team Meetings, coordinating with families, service providers, and other stakeholders to develop and implement individualized plans of care.
  • Provide ongoing support and advocacy for youth and families, connecting them with community resources and services as needed.
  • Collaborate with other program staff, including consulting Psychologists and Psychiatrists, to ensure comprehensive and coordinated care.
  • Document all interactions and progress in a timely and accurate manner, maintaining up-to-date records and reports.
  • Participate in regular training and professional development opportunities to enhance skills and knowledge.

Requirements:

  • Bachelor's degree in a relevant field, such as social work, psychology, or human services.
  • Minimum of one year of experience providing mental health services, preferably in a Wraparound or similar model.
  • Working knowledge of computer applications, including Microsoft Word and Excel.
  • Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities.
  • Strong communication and interpersonal skills, with the ability to build rapport with diverse populations.
  • Valid driver's license, automobile, and insurance sufficient to meet agency requirements.

Working Conditions:

This position requires regular travel to meet with families and service providers, as well as occasional evening and weekend work. The successful candidate will be able to work effectively in a busy office environment, managing multiple responsibilities and priorities.
