Job Title: Care Coordinator

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Lutheran Social Services of WI and Upper MI. As a Care Coordinator, you will play a vital role in supporting children with developmental, physical, or severe emotional disabilities and their families.

Job Summary:

The Care Coordinator will serve as the main point of contact for families, helping them navigate the Children's Long Term Support (CLTS) Waiver program. This federally funded Medicaid program provides essential services to children with disabilities, enabling them to live safely in their homes and communities.

Key Responsibilities:
  • Assess the needs of children and their families, utilizing required assessment tools to develop individualized service plans.
  • Work collaboratively with clients, service teams, and clinical supervisors to determine interventions and implement service plans.
  • Coordinate and facilitate person-centered team meetings, ensuring effective communication and collaboration among stakeholders.
  • Provide services to assigned clients, aligning with service plans and promoting client independence and growth.
  • Document services provided, maintaining accurate and timely records.
Requirements:
  • Bachelor's Degree in Social Work or a related field, with a minimum of one year of experience working with children and/or youth with disabilities.
  • Valid driver's license and reliable transportation, with a satisfactory driving record.
  • Ability to work in a community environment, with occasional travel and exposure to outdoor conditions.
