Job Summary

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Wisconsin Community Services, Inc. As a Care Coordinator, you will play a critical role in providing intensive and individualized care coordination for children and adolescents under the Wraparound Practice Model.

Key Responsibilities
  • Maintain a caseload of approximately 8 children and families, providing regular visits, phone contact, and support as needed.
  • Assemble a Child and Family Team within two weeks of enrollment, identifying family members, natural supports, and service providers to develop an individualized Plan of Care.
  • Coordinate the meeting of the Child and Family Team and develop the initial Plan of Care based on identified strengths and needs.
  • Monitor the provision and quality of services provided to the family, advocating for their needs across various settings.
  • Attend all scheduled court hearings as a representative of Wraparound Milwaukee to support children and families throughout the court process.
  • Seek community resources with the assistance of the Team and modify the Service Agreement (SAR) as needed.
  • Provide or arrange for transportation for youth/young adults to appointments, crisis/respite services, and assist with identifying natural supports and sustainable transportation plans.
  • Complete all necessary paperwork in a strength-based manner, including court letters, Plans of Care, change of placement forms, SARs, Referrals, Progress Notes, and evaluation instruments.
  • Maintain accurate information online, ensuring that family demographic information and Child and Family Team Member contact information is up-to-date.
Requirements
  • Bachelor's degree in a relevant area of education or human services and a minimum of one year of continued experience providing mental health services.
  • Complete 85 hours of mandatory training in Wraparound philosophy and policies, as well as attend a Family Orientation within 6 months of hire date.
  • Working knowledge of computer applications, including Windows and Microsoft Word.
  • Ability to display cultural competence and respond respectfully to people of all cultures, languages, classes, races, ethnic backgrounds, religions, and other diversity factors.
  • Valid driver's license, automobile, and insurance sufficient to meet agency requirements.
Working Conditions

The work environment is primarily in a busy office setting, managing caseloads of children, young adults, and families. Some outreach is done in the community and families' homes, with occasional evening and weekend work required.
