Key Responsibilities- Maintain a caseload of approximately 8 children and families, providing regular visits, phone contact, and support as needed.
- Assemble a Child and Family Team within two weeks of enrollment, identifying family members, natural supports, and service providers to develop an individualized Plan of Care.
- Coordinate the meeting of the Child and Family Team and develop the initial Plan of Care, including a comprehensive 24-hour Crisis Plan.
- Monitor the provision and quality of services provided to the family, advocating for their needs and liaising with service providers as necessary.
- Attend all scheduled court hearings as a representative of Wraparound Milwaukee, supporting children and families throughout the court process.
- Seek community resources and modify the Service Agreement (SAR) as needed, providing or arranging for transportation and support services for youth and families.
- Complete all necessary paperwork in a strength-based manner, maintaining accurate information online and collaborating with other necessary individuals.
- Bachelor's degree in a relevant area of education or human services, with a minimum of one year of continued experience providing mental health services.
- Complete 85 hours of mandatory training in Wraparound philosophy and policies, as well as attend a Family Orientation within six months of hire date.
- Working knowledge of computer applications, including Windows and Microsoft Word.
- Ability to display cultural competence and work effectively with diverse populations.
- Valid driver's license, automobile, and insurance sufficient to meet agency requirements.
The work environment is primarily in a busy office setting, managing caseloads of children, young adults, and families. Some outreach is done in the community and families' homes, with occasional evening and weekend hours.
